Instructional League Registration Form

This year—you received a notice in the mail, and your child may have brought home a copy of the same notice—informing you that you had to register for SYAC in person.

This was done because of an overwhelming request by parents after the 2007 season to have more direct contact with the "management" of the SYAC organization. It was also done to facilitate the fitting of uniform shorts that were equally requested by parents and to meet with us if you are interested in coaching a team.

Instructional League Registration is scheduled for Monday, January 5, 2009 at the H.B. Thompson Middle School Cafeteria. If this night is not convenient for you, we don't want you to feel like you have missed out on the season. You can still come down on any of the other scheduled nights, or you can use the form below to register by mail. If you choose to register in person, your child must be accompanied by his or her own parent.

A note about unform size selection
This year, for the first time, we are providing a full uniform (i.e., hat, shirt, shorts) for the Instructional League as we do for all the other age groups. On the form below, you will find a blank in which you need to write a Uniform Size. Shirts and hats are all one size, but shorts are different. Please enter the appropriate "Size Number" from the table below, which explains what each size number means for the shorts:

The fee for Instructional League registration is $125.00, payable by check (by mail or in person) or cash (in person only).
